CDR Worksheets

These translations made possible through the generous efforts of MAPI Research (translation certificate) and Corporate Translations (translation certificate). Each translation includes the CDR semi-structured interview in worksheet form and the CDR scoring table. The scoring rules for global CDR determination are available in English only (Click here to view). Please consider using our online scoring algorithm to ensure an accurate final CDR rating.

The materials provided below are intended for clinical and research use. We strongly recommend individuals using CDR materials apply for training via our Online CDR Training System. All files below are in Adobe Acrobat PDF format. In addition, a link to the CDR Scoring Algorithm is also provided.

INSTRUCTIONS:

(1) PRINT ONLY: By left-clicking on any hyperlinked file on this page, the PDF will load in a new window. Click on the printer icon that appears in the Acrobat PDF toolbar.

If you click the link at the bottom of this page allowing you to view all of the translations, every link on the page that appears will load in that same window window. Click on the printer icon that appears in the Acrobat PDF toolbar print the PDF. You will then need to hit the back arrow in your browser to return to the menu choices.

(2) DOWNLOAD: Right-click on the translation you wish to download. Left-click on "Save Target As" or "Save Link As". Choose the download location on your computer you wish to save the translation to. Finally, left-click on "Save" to download the translation to your computer
