PURCHASE CDR VIDEOTAPES AND CD-ROMs

Training videotapes and print materials for the Clinical Dementia Rating (CDR) are available for purchase. We accept purchase requests via e-mail only (see below). We usually bill at the time of shipment, but pre-payment is also possible. We prefer payment by check in US Dollars, but we will accept an institutional purchase order or credit card. How do I place an order?

We currently provide online CDR training and a fee schedule for commercial users of the CDR (e.g., Pharmaceutical Companies). Training options will include contracting for an ADRC-trained clinician to provide CDR training at Investigator Meetings and for online training and reliability testing protocols for study personnel to access from home or office. We will continue to make available our standard Brief Training & Reliability Protocol for purchase as described below. What training protocols are available for purchase?

  • Brief Training & Reliability Protocol (BTRP) - This is our standard training and reliability protocol for general CDR training. This protocol consists of 10 videotaped segments running ~35 minutes each. The first video segment provides an overview of the CDR from Dr. John C. Morris, Co-Director of the Washington University Alzheimer's Disease Research Center. The remaining nine are real patient interviews conducted at Washington University (see Copyright and Dissemination Policy). Written documentation includes the CDR worksheet, scoring table, scoring instructions, and applicable references. Total time to complete the training is 6-8 hours. Four formats are available currently: Region Free NSTC DVD ($300 US), Standard VHS Videotapes ($300 US), VHS-PAL Videotapes ($400 US), and a CD-ROM version ($300 US). Shipping costs are extra.
  • ADCS MCI Protocol - This specialized protocol focusing on early-mild dementia was developed for the Alzheimer's Disease Cooperative Study. It consists of 4 videotapes and takes 2-3 hours to complete. Only Standard VHS videotapes are available. This protocol is distributed to approved ADCS sites only.
  • ADCS Standard Protocol - A shortened version of the BTRP. This protocol developed for the Alzheimer's Disease Cooperative Study. It consists of 6 videotapes and takes 3-4 hours to complete. Only Standard VHS videotapes are available. This protocol is distributed to approved ADCS sites only.
  • ADCS PI Study Protocol - A specialized protocol for the ADCS Prevention Instrument Trial. It consists of 1 videotape or CD-ROM. This protocol is distributed to approved ADCS sites only.
  • ADCS Homocysteine Study Protocol - A specialized protocol for the ADCS Homocysteine Trial. It consists of 1 videotape or CD-ROM. This protocol is distributed to approved ADCS sites only.

What information should my purchase request include?

Send your purchase request via e-mail to webmaster@abraxas.wustl.edu. Your request should include the information listed below. Requests that do not include required information (denoted by *) may lead to a delay in processing. Orders are usually shipped within 7 business days of the request via UPS or Federal Express. An invoice with the order includes an additional shipping charge.

Include the following information with your purchase request:

(4) Full mailing address for shipment and billing (include both if different) *

(7) Please include a statement describing your interest in ordering and using the CDR. Include whether your use is for clinical, research or educational purposes. *

(8) If you are purchasing the CDR protocol for research, please include a brief abstract about the study. *

(9) If you are purchasing the CDR protocol for a Clinical Drug Trial, please include the name and affiliation of the Principal Investigator, name(s) of sponsoring the company (companies), name of agent and/or trial number, and anticipated start date.*

(10) ADCS Requests need only list the study title and PI name.

(11) What do you intend as your method of payment? A check in US Dollars is the preferred form, but institutional PO and credit card payment is also possible. *
